Blood Assurance is
dedicated to serving the needs of our community and to helping people display
kindness and generosity through giving the Gift of Life. This scholarship fund is dedicated to Crystal
Green, who was a valiant and loving young woman who set forth the example of
kindness and generosity to all she met.

In her honor, the Blood Assurance Foundation, Inc., established a scholarship fund in 1999 for high school students seeking to better themselves and give back to their communities. The program offers a total of twelve (1) one-year scholarships in the amount of $1,500 each. The scholarship is paid directly to each student’s college or university for tuition, fees, and other educational expenses. Based on the institution’s academic schedule, scholarship money will be paid on a semester or quarterly basis.
Applicants
will be judged on a written application, high school transcript, school and
community service, letters of recommendation, and a marketing plan for a blood
drive. The time frame for consideration is critical. The completed applications
must be postmarked by March 31st. Students must attend a school that actively
participates in “Project Lifeline,” the school blood drive program offered by
Blood Assurance.
Final
selections are determined by the Scholarship Committee by the first week of
May. All decisions are final. During the summer prior to entering as a college
freshman, each recipient must confirm his/her status for the coming academic
year.
At
Blood Assurance, we believe that education is the key to
Eligibility:
1. You must be a high school
senior planning to enter an accredited two or four-year college or university
the following fall. It must be a public, or private, not-for-profit institution
from which you can receive an associate or bachelor’s degree.
2. You must have at least a “B”
average (equivalent GPA) and score at least 20 on the ACT or 1100 on the SAT
(cumulative). The College Board revised sections and scores for the SAT in
2005, however the SAT score requirements for the Crystal Green Memorial
Scholarship have not changed.
3. No applicant will be awarded
more than one Crystal Green Memorial Scholarship and the scholarship is
non-renewable.
4. Blood Assurance reserves the
right not to award the scholarships if no applicant achieves the minimum
standards. In addition, Blood Assurance
reserves the right to discontinue this scholarship program, provided that any
scholarship already awarded will continue, subject to the provisions of the
section titled “Maintaining the scholarship.”
5. Financial need is a
consideration. Applicants who have all educational expenses paid through other
scholarships and/or grants are ineligible.
1. Applicants must be high school
seniors. Students do not have to be blood donors, but donors will be given
special consideration.
2. Responses to all questions
should be typed and postmarked by March 31st.
3. Write a detailed marketing
plan for a new blood drive promotion.

4. Secure two letters of
recommendation. Neither can be from a relative.
5. Obtain an official high
school transcript which includes your first semester senior grades and your
ACT/SAT test scores.
6. If you would like Blood
Assurance to acknowledge receipt of your application materials, attach a
self-addressed, stamped envelope to the application.
7. All scholarship recipients
will be notified in May. To receive a
copy of the list of recipients, send a self-address, stamped envelope to the
address below after May 30th.
Judging:
1. Our judges will review all
applications. The judges will consider all
aspects of the application process- the essay, transcript, school and community
service, letters of recommendation, and a personal interview with the student
finalists- and choose our scholarship recipients. The judges’ decisions will be
final.
2. Any application containing
falsified information will be immediately disqualified.
Maintaining the scholarship:
1. If you are awarded a
scholarship, you must begin attending your college or university the following
fall. The money ($1500.00 total) will be
paid at the beginning of the semester or quarter. You must be enrolled
full-time and maintain at least a “C” average (2.5 on 4.0 scale or 3.75 on 5.0
scale) at the end of each academic semester/quarter (Summer school will be
included in the academic year, but scholarship money will only pay for
traditional fall/spring semesters or fall/winter/spring quarter). You must have
the school send us a transcript at the end of each semester/quarter.
2. Upon our request, you must
be available to present a written or oral report on your academic progress.
3. If you withdraw for any
reason, you will no longer be eligible to receive the scholarship.
4. If you lose the scholarship
for failure to meet any requirements, you will not be able to re-qualify.

IV. Write a detailed marketing plan for a promotional blood drive event of your design. Please include a one page summary and analysis of your plan, followed by a detailed plan of attack for your program in the following areas:

§ What promotional materials would you use? T-shirts?
Hats? Other giveaways?
§ What designs would you use?
§ How would you utilize the media to get the word out?
§ Could you tie the promotion into some other event?
§ Where would you send the bloodmobiles?
§ How will you involve Blood Assurance employees?
§ How will you motivate donors to participate?
§ And any other pertinent details.
Remember, this is your promotion. Be as detailed as possible regarding how you would pull it off. If your plan is a “knock-your-socks-off” work of creative genius, you may win a scholarship and actually have your plan used to save lives!
